The main objective of the Dorchester County School District Two Health Program is to assist each student in achieving and maintaining optimal health so that maximal physical, social, emotional, and academic growth can occur. 

• School Exclusion List from DHEC ~ When should your chid stay home? Please read the information from DHEC in English or Spanish.

• Student Health Information Card ~ Please complete a health information card for your student to keep in the nurse's office. Click here to print a copy. 

• Medication At School ~ Please register all medication with the nurse's office, including over the counter medication. 

• Student Self Carry Forms: Parent Authorization Form and Health Care Practitioner Authorization Form

• Medicine to be Held in Nurse's Office: Medication Permission Form and Over the Counter Guidelines 

• Immunizations

Parents will be notified by the school nurse if immunization records are missing or needed.
