1. PlanetHS Parent & Student-Athlete Accounts
Both student-athletes, and a parent/guardian, will need a PlanetHS account. You can access the PlanetHS website by clicking the link to the right. PlanetHS Website Link.
All required participation forms must be uploaded and submitted through PlanetHS for approval in order to participate in GMS athletics. Students without a PlanetHS account, or who have not submitted all required forms through their PlanetHS account will not be allowed to participate in GMS athletics.

2. Medical History Form
All student-athletes must have a completed medical history form uploaded into their PlanetHS account. You can find the form HERE. These must be completed by the examining physician.
3. Physical Examination Form
All student-athletes must have a physical examination performed by a physician. You can find the physical form HERE. These must also be uploaded into a student's PlanetHS account. *No student will be allowed to try-out, practice, or participate in any GMS athletics without a physical. Please ensure that the physician's signature & practice name is CLEARLY visible when uploading this document.
