  • A Note from the Nurse

    Hello, Eagle Families! 

    We could not be more excited to welcome you and your student to OMS for another successful year as an Oakbrook Eagle! Our main priority in the OMS Health Clinic is ensuring that your student is able to get the most out of their educational experience by keeping them safe and healthy while at school. We aim to provide a safe and welcoming space for your student. If there is a medication that your student needs daily or on an as-needed basis at school, please see the below documents in order to start that process. Please contact us if you have any questions throughout the school year, and we will respond as soon as possible. Our goal this year is to give all of our Eagles the tools and skills they need to navigate today's healthcare environment with confidence and self-advocacy.

    Immunizations

    It is a South Carolina law that all students enrolled in public schools must maintain a valid South Carolina Certificate of Immunization (Dhec Form 4024). All students must comply with SC Code 44-29-180 and SC Regulation 61-8. All vaccinations can be administered by your local physician, any DHEC Health Clinic, or any other provider of pediatric vaccines. A valid SC Certificate of Immunization must be provided to Nurse Gooch by Thursday, September 15, 2022, or within 30 days of enrollment for new students.

    Exclusion from School

    In an effort to help protect well children from unnecessary exposure to contagious or infectious diseases, South Carolina requires that children and staff with certain diseases and conditions stay home from school or childcare while contagious.
