1095-C Update

Important Update About Your Health Coverage Tax Document

Following the Paperwork Burden Reduction Act, Dorchester District Two now offers 1095-C forms electronically to all active, full-time employees via Employee Access. In accordance with this act, the district will no longer automatically provide paper 1095-C forms.

How to Request a Free Paper Copy

If you prefer a physical copy, you may request one through any of the following options:

Important Reminders

  • Processing Time: Once your request is received, the form will be mailed to your address on file within 30 days.
  • Address Verification: Please double-check your mailing address before requesting. You can update your address through Employee Access or by contacting the Personnel Department.

Please note that Form 1095-C is not required to file a federal income tax return.
