Organizations should complete KF-E(2), KF-E(3), Fee Schedule, and the Fee Waiver Request (if necessary) and submit to the school at least 15 business days prior to the first requested usage. The Principal or designee should review the application, availability of facility, and request any additional or clarifying information (such as
staff for audio/visual/scoreboard). Once approved by the Principal, these documents should be submitted to Jenene Gourdine in Curriculum and Instruction for District Level approval. The requesting organization must submit the required insurance documentation and make payment prior to use.
